Dual Echo Fast Gradient EchoInfoSheet: - Sequences -
Intro,
Overview,

(DE FGRE, Dual/FFE, DE FFE) Simultaneously acquired in and out of phase TE gradient echo images. To quantitatively measure the signal intensity differences between out of phase and in phase images the parameters should be the same except for the TE.
The chemical shift artifact appearing on the out-of-phase image allows for the detection of lipids in the liver or adrenal gland, such as diffuse fatty infiltration, focal fatty infiltration, focal fatty sparing, benign adrenocortical masses and intracellular lipids within a hepatocellar neoplasm, where spin echo and fat suppression techniques are not as sensitive. Specific pathologies that have been reported include liver lipoma, angiomyolipoma, myelolipoma, metastatic liposarcoma, teratocarcinoma, melanoma, haemorrhagic neoplasm and metastatic choriocarcinoma.

In MRI, an echo is the emission of energy in form of an electromagnetic resonance signal of a nuclei after its excitation. At this point spins are back in phase again and the signal is measured. The desired number of echoes is selectable. Often until eight echoes are permissible for 2D or 3D scans using spin echo, inversion recovery or MIX techniques. Two echoes are permissible for all other techniques. A multi echo imaging sequence is needed for simultaneous measurement of T2 and density weighted images.

Echo Offset

Echo offset is the time setting of spin echo and gradient echo to be not coincident and to generate phase differences between different spectral line signals (e.g., water and fat). The echo offset is the product of the frequency line difference and the time difference (TD) in the echo times and is equal to the magnitude of the result of the phase difference between two spectral lines. Phases may not change linearly with echo offset time in the presence of a large field inhomogeneity. An echo offset excitation pulse sequence can be used in the magnetic field mapping method, to generate maps from which the standard deviation of the phase difference can be calculated.

FerumoxideInfoSheet: - Contrast Agents -
Intro, Overview,
Characteristics,
Types of,
etc.

Short name: AMI-25, generic name: Ferumoxide (SPIO)
Ferumoxides are superparamagnetic (T2*) MRI contrast agents, so the largest signal change is on T2 and T2* weighted images.
The agent distributes relatively rapidly to organs with reticuloendothelial cells primarily the liver, spleen and bone marrow. The liver shows decreased signal intensity, as does the spleen and marrow. The agent is taken up by the normal liver, resulting in increased CNR between tumor and normal liver. Hepatocellular lesions, such as adenoma or focal nodular hyperplasia, contain reticuloendothelial cells, so they will behave similar to the liver, with decreased signal on T2 weighted images. On T1 images, there is typically some circulating contrast agent, and blood vessels show increased signal intensity.
Current MRI protocols involve T1 weighted breath-hold gradient echo images of the liver, and fast spin echo T2 weighted pictures. This requires about 15 minutes. The patient is then removed from the scanner, and the contrast agent administered. After contrast administration, the same pulse sequences are again repeated.

Fetal MRI

Ultrasound imaging is the primary fetal monitoring modality during pregnancy, nevertheless fetal MRI is increasingly used to image anatomical regions and structures difficult to see with sonography. Given its long record of safety, utility, and cost-effectiveness, ultrasound will remain the modality of first choice in fetal screening. However, MRI is beginning to fill a niche in situations where ultrasound does not provide enough information to diagnose abnormalities before the baby's birth. Magnetic resonance imaging of the fetus provides multiplanar views also in sub-optimal positions, better characterization of anatomic details of e.g. the fetal brain, and information for planning the mode of delivery and airway management at birth.

Indications:
Fetal anomalies
Maternal tumors
Pelvimetry
Examinations of the placenta

Modern fetal MRI requires no sedatives or muscle relaxants to control fetal movement. Ultrafast MRI techniques (e.g., single shot techniques like Half Fourier Acquisition Single shot Turbo spin Echo HASTE) enable images to be acquired in less than one second to eliminate fetal motion. Such technology has led to increased usage of fetal MRI, which can lead to earlier diagnosis of conditions affecting the baby and has proven useful in planning fetal surgery and designing postnatal treatments. As MR technology continues to improve, more advances in the prenatal diagnosis and treatment of fetal abnormalities are to expect. More advances in in-utero interventions are likely as well. Eventually, fetal MRI may replace even some prenatal tests that require invasive procedures such as amniocentesis.
